Where do I start?

1. Call 1-800-HENRYFORD (436-7936). You will be connected to staff dedicated to assisting Canadian patients. They will schedule a consultative appointment with a physician who specializes in your condition.  
2. At the consult, our care team will discuss your condition and treatment options with you.  
3. If you are a candidate for a surgical procedure and wish to proceed, you will meet with a Care Coordinator who will explain the process of obtaining Ministry of Health approval and provide information about the procedure.
4. Your Canadian family physician will need to complete a "Prior Approval Application for Full Payment of Insured Out-of-Country Health Services" form and submit it to the Ministry of Health.
5. The Ministry of Health will contact us for information about the procedure and cost.  
6. If the Ministry of Health approves your request for services, your Care Coordinator will contact you to schedule a procedure date.
7. If the Ministry of Health does not approve your procedure, you will have the option of paying for the procedure yourself. Or your Henry Ford specialist will work with your family practice doctor to devise a treatment plan for your care in Canada.

Can I afford care at Henry Ford?
The most common cost associated with a consultation appointment is between $211 US and $282 US, depending on your condition and the amount of time you spend with the physician. For any services paid out-of-pocket, you will receive a 40% discount. Henry Ford accepts cash, credit card or cashier's check. Payment will be collected when you check in for your appointment.

To have your medical care paid for by Ontario Ministry of Health, your primary care doctor will need to complete a Prior Approval Application for Full Payment of Insured Out-of-Country Health Services form. 

If the services you are requesting are not covered by your insurance, you still may have the procedure, however, all charges for the procedure will need to be paid by you, in full, prior to your procedure date.
