- Early identification of children with chronic diseases such as asthma and diabetes, and ongoing medical management to prevent complications of these diseases in children
- Early identification of children with developmental disabilities, and ongoing medical management to prevent development of secondary medical conditions in these children
- Health promotion for children with disabilities
- Transition to adult life and medical care
