Sports Physicals
Presented by Henry Ford Jackson Sports Medicine
Henry Ford Sports Medicine - Jackson is excited to conduct sports physicals for students planning to participate in athletics during the 2024–2025 school year. All locations are open to students from any school district needing a sports physical. No appointment is necessary.
Students must bring:
- Sports physical (download) and waiver forms (download) completed and signed by a parent.
- Cash payment of $20. Payment is accepted at the time of the sports physical. Please note, athletes who are not accompanied by parents can only be seen by the physician if all forms are completely signed.
Please print these forms and have them completed prior to attending the sports physical. The physicals cannot be conducted without these forms.
