What is Mosaic ACO?
Mosaic Accountable Care Organization (ACO) is part of the Medicare Shared Savings Program through the Centers for Medicare & Medicaid Services (CMS). It brings providers and hospitals together to improve coordination, avoid unnecessary services and reduce medical errors.
When an ACO delivers high-quality care while lowering total spending, it may share in the savings it generates for the Medicare program.
What this means for you
If your primary care provider participates in Mosaic ACO, you may see benefits such as:
- Better coordination across your care team
- More proactive outreach for prevention and chronic condition support
- Fewer repeated forms, tests or gaps in information when care is shared across settings
Your Medicare coverage and choices stay the same
Being served by an ACO is not the same as Medicare Advantage. ACOs follow Original Medicare rules.
- You can still see any provider or hospital that accepts Original Medicare
- You keep the same Original Medicare benefits and protections
- There is no additional premium to be in an ACO, and there is no requirement that you receive services from ACO providers
How your information may be used
To support care coordination and quality improvement, Mosaic ACO may request Medicare claims data about your care. CMS requires beneficiaries be told they can decline data sharing.
Want to decline claims data sharing?
You can ask CMS not to share your claims data with an ACO by contacting 1-800-MEDICARE (1-800-633-4227).
Choosing your primary clinician (voluntary alignment)
CMS also allows Medicare fee-for-service beneficiaries to select their primary provider on Medicare.gov.
