What is Mosaic ACO?
Mosaic Accountable Care Organization (ACO) participates in the CMS Medicare Shared Savings Program, which brings providers and hospitals together to deliver coordinated, high-quality care for people with Medicare.
When an ACO performs well on quality and lowers total cost of care, CMS may award a performance payment (shared savings).
What Mosaic ACO focuses on
Mosaic ACO supports high-value care across the network by focusing on practical drivers of outcomes and performance:
- Chronic disease management, including diabetes and hypertension
- Fewer avoidable readmissions and unnecessary hospital stays
- Stronger prevention, including cancer screenings and immunizations
- Accurate HCC documentation and coding to reflect patient complexity and support appropriate care
- Care coordination across settings to help patients stay healthier and at home
Provider resources and engagement
These are the areas that most often drive results:
- Preventive care workflows: screenings, immunizations, wellness visits
- Risk and coding accuracy: problem list, specificity, annual assessment, HCC capture
- Transitions of care: timely follow-up after ED visits and hospitalizations
- Closing care gaps through consistent, team-based outreach and follow-through
