Self-Reporting Your Sexual Orientation and Gender Identity (SOGI)

At Henry Ford, our number one priority is making you feel safe and comfortable when you receive care at any of our facilities. We strive to create a welcoming environment where all patients can be their most authentic selves.

That is why all Henry Ford patients can self-report their sexual orientation and gender identity (SOGI) information from their MyChart account or when they visit any Henry Ford healthcare provider. 

Self-reporting in MyChart

From your MyChart account, you can update the following information:

• Preferred name
• Legal sex
• Gender identity
• Sexual orientation

With the exception of sex assigned at birth, you can change this information at any time.

Your privacy matters

This information is stored in your Henry Ford medical record. It can only be seen by those care providers who are directly involved in your care and a few other staff members who may need it for billing or legal reasons. Your information is confidential and protected by law, just like all your other health information.
