What the Henry Ford Joint Venture with Ascension Michigan Means to You
Henry Ford Health and Ascension Michigan entered into a joint venture agreement on Oct. 1, 2024. At this time, Ascension providers and facilities are not part of the ConnectedCare: Henry Ford Health network.
In partnership with GM, Henry Ford Health will carefully select Ascension Michigan providers, medical centers, hospitals and other resources that will serve you best. This process will take time, and you will be notified of changes to your ConnectedCare: Henry Ford Health Level 1 provider network as updates occur.
For now, your Level 1 provider network remains the same, with access to more than 2,800 Henry Ford Health affiliated providers.

If you are new to Henry Ford Health and you don’t currently have a relationship with a non-Henry Ford Health provider, call the concierge team at (844) 436-7637 to select a primary care physician and/or specialist and schedule an appointment. To find providers in Level 1, visit BCBSM Find a Doc.

Transition care as a new enrollee to Henry Ford Health
If you are new to Henry Ford Health and you currently have a relationship with a non-Henry Ford Health provider(s), call the concierge team at (844) 436-7637 to discuss your existing, non-Henry Ford Health providers and to find Henry Ford Health primary care or specialty providers that match your needs. The concierge team can work with you to schedule an appointment to establish care with your new provider, as well as explain what is needed to transfer your medical records to your new Henry Ford provider, if needed.
