2024 Program Requirements

Complete the Enrollment Waiver and WebMD Online Personal Health Assessment from March 18th – April 5th.

Biometric Health Screening

Your enrollment email from It’s Your Life will indicate if you are required to have a health screen this year.

Option 1: Onsite health screen at GLI on March 27th, March 28th, April 1st and April 2nd. The sign up will be posted on the bulletin board. You are required to fast for at least 12 hours before (water and medications are allowed). You will be excused from blood work if pregnant or nursing.

Option 2: Have your screening done at your physician's office and return the Physician's Results Form to IYL by fax. We will accept blood pressure, weight and height values done since January 1, 2023, and cholesterol and blood sugar values since January 1, 2022. The deadline to return this form is April 5th, 2024. A Physician’s Results Form will be attached in the enrollment email from IYL.

Participate in Health Coaching Sessions

Health coaching sessions will be held in April, July, and October. Instructions to sign up for coaching sessions will be sent via email.
