Provider-Based Billing: What You Need To Know
If you see two fees on your Henry Ford Health billing statement, that’s provider-based billing. It means your care happened at a hospital-owned location, even if it’s not inside the hospital.
Why two fees?
Henry Ford is a hospital-based health system which means that bills are split into two parts for most of our locations – doctor and facility. Like most services, they charge you for the materials and labor. For Henry Ford, labor is the doctor and materials are the facility. That’s why you’ll see these two separate fees on your billing statement:
- Professional fee: For the doctor or healthcare professional who provides you care
- Facility fee: For the location resources, staff and equipment used during your visit
Does this affect me?
Provider-based billing can impact out-of-pocket costs for most patients, depending on your insurance. Check with your insurer to confirm your out-of-pocket costs.
Medicare billing notice
Starting Dec. 9, 2025, Medicare patients at Henry Ford provider-based locations must acknowledge a coinsurance notice during eCheck-In or at the front desk.
Why this notice?
Medicare requires patients at provider-based locations receive this notice before care. If you get this notice, that means you may receive two separate coinsurances — one for professional services and one for facility services.
This is not a new policy but a change in how we communicate this information to patients. Henry Ford Health has always billed this way.
Who does this apply to?
Patients with fee-for-service (FFS) Medicare, including those with Medicaid as secondary insurance, receiving care at Henry Ford outpatient hospital departments that are designated provider-based sites.

Can I get an estimate for my coinsurance payment?
Below is an idea of what you might expect your facility fees to be (this does not include professional fees). This can change based on the services you get and your insurance coverage:
- Clinic visits: $10
- Clinic procedure: $35 to $50
We also provide typical coinsurance amounts for commonly performed procedures in our provider-based facilities. If you have supplemental insurance, your coinsurance costs are usually paid by that insurance.
