Transition care as a new enrollee to Henry Ford Health

If you are new to Henry Ford Health and currently have a relationship with a non-Henry Ford Health provider, call the concierge team at (844) 436-7637 to discuss your current providers and find Henry Ford Health primary care or specialty providers who meet your needs. The concierge team can help you schedule an appointment to establish care and explain how to transfer your medical records, if needed.

Call 911 or go to the nearest emergency room if you are experiencing chest pain, shortness of breath, numbness, weakness, loss of vision, uncontrolled bleeding, bone poking through skin, swollen tongue, or severe burns.

Qualified emergency care is covered at the in-network benefit level at any emergency room, including but not limited to Henry Ford Health emergency rooms.
