Welcome to Henry Ford Health! We are thrilled to have you join the Henry Ford family. Before your start date, it is important to complete the tasks outlined in sections below. To simplify the process, we have organized these tasks in a sequential order. We kindly request that you carefully read each step as it provides clear instructions on what needs to be done. Your prompt attention to these tasks is highly appreciated, and we sincerely thank you for your cooperation.

New Hire Onboarding Tasks


  • Part 1: Mandatory Pre-Hire Tasks

    Please complete tasks within 2 days after accepting your offer. It should take approximately 20 minutes to complete these tasks.

    • Navigate to the careers page and log in with the username and the password created when you applied for the position. Please contact the Onboarding and Retention team if you have trouble logging in.
    • Review welcome letter from President and CEO, Robert Riney – yep, even Bob Riney is excited you’re part of the Henry Ford Health team!
    • Review and submit Personal Information Form. The name used should be your legal name.
    • Complete EEO & Veterans Status Forms
    • Complete Mandatory Arbitration Agreement – you will receive an email and the subject line will say ‘Notification: Mandatory Arbitration Form’
    • Consent to Background Check – after consenting, you will receive an email from Corporate Screening, subject line will say ‘Review & Complete: Additional Information Required’
    • Complete requested background check information for Corporate Screening.

  • Part 2: Required Tasks for Clearance to Start: Employee Physical, Learning Modules, Form I-9, and BLS Certification

    You should have received an email with your scheduled date and time for your preplacement physical and onboarding appointments. It's crucial you make every effort to attend your appointment on the original scheduled date. Rescheduling the appointment may cause a delay in your start date. If you do need to reschedule your appointment, please contact the Onboarding and Retention team. You will also receive a reminder call from Employee Health the day prior; please make every effort to answer their call.

    New Employment Physical and Onboarding Appointment

    Refer to your email to confirm the date, time, and location of your preplacement physical and onboarding appointment, as well as the required documents to bring. You will receive an email from Employee Health notifying you if you have passed your physical. In addition, Employee Health will let you know if an additional appointment is necessary for additional follow-up.

    Immediately following your preplacement physical, you will meet with Onboarding to complete your I-9. Please refer to the instructions below for assistance in completing your I-9 prior to attending your preplacement physical.

    Learning Modules and Form I-9: These tasks should be completed 7 days PRIOR to your start date.

    Within 30 days of your start date, you can expect your New Employee Letter. This email is essential for completing your modules and Form I-9. Rest assured, you will receive this email within the specified timeframe. However, if your start date is 30 days away from today and you haven’t received the email, we kindly request you reach out to our Onboarding and Retention team for assistance.

    • Locate your New Employee Letter.
    • Create password to access Employee Self Service. Contact Onboarding and Retention if you have any issues logging into Employee Self Service.
    • Pre-hire learning modules:
      • You will find your assigned learning modules in the Henry Ford University. Completion of learning modules is a requirement to be cleared to start and must be completed by the Wednesday prior to your start date. A tip sheet is included for your reference.
    • I-9 Information:
      • Before attending your new employee physical, complete the Employee Information and Attestation section of your I-9.
      • Bring original (copies cannot be accepted) forms of employment eligibility documents to Onboarding for physical examination. See a list of acceptable documents here.
      • Your I9 must be completed prior to your start date.


    EMPLOYEES THAT LIVE OUT OF STATE: The following directions are applicable ONLY for hires that live out of state.

    • Step 1: Access the Employee Self Service portal to complete the Employee Information and Attestation section of your Form I-9 online. Please refer to the steps below for guidance. If you have already completed this step, proceed to step 2.
    • Step 2: Download a paper Form I-9 here and complete.
    • Step 3: Designate an authorized representative to complete section 2 of your Form I-9 on behalf of Henry Ford Health. The authorized representative must physically examine, with the employee being physically present, each document presented to determine if it reasonably appears to be genuine and relates to the employee presenting it. Reviewing or examining documents via webcam is not permissible. An authorized representative can include personnel officers, foremen, agents or notary public; this does not include family members.     • Step 4: Email your completed Form I-9 to HROnboarding@hfhs.org

    BLS Certification: Please complete 7 days PRIOR to your start date.

    If you have been hired as a Registered Nurse, Medical Assistant, Nurse Assistant, Nurse Extern, Nurse Resident, Patient Safety Assistant, or LPN: Please complete the BLS certification information.

  • Part 3: Badging Information

    Your badge picture will be taken at the completion of your new employee physical, except for employees working at the Henry Ford Jackson and Optimeyes locations.

    • If you are working at HENRY FORD JACKSON: Your badge picture will be taken on your start date.
    • If you are working at HENRY FORD OPTIMEYES: You will receive badging instructions from your location at a later time.
  • Part 4: Post-Hire Tasks

    These tasks should be completed within 3 days AFTER your start date. It should take approximately 45 minutes to complete these tasks.

    Log into Employee Self Service and begin working through your Onboarding Checklist.

    Some things to note:

    • When electing your W-4 withholding, be sure to thoroughly review the W4 election page before finalizing your W4 election! We HIGHLY recommend speaking to a tax advisor before claiming Exempt from federal tax withholding.
    • Ensure you have correctly entered your bank account and routing numbers when electing Direct Deposit.
    • Visit My Choice Rewards for detailed information regarding Health and Welfare plans.
    • Contact Employee Services at 1-855-874-7100 or EmployeeServices@hfhs.org for questions regarding your benefits, direct deposit, or W-4 election.
