Working closely with your physician, our compassionate At Home team will personalize a care plan to fit your needs.

The telemonitoring program helps patients manage chronic conditions, such as heart failure and COPD, and helps prevent visits to the emergency department and hospitalizations.

Home Medical Equipment

Henry Ford Allegiance Home Medical Equipment enhances independence, comfort and safety at home. We provide 24-hour emergency service, home delivery and instruction on how to use the equipment.

Advanced Illness Management (AIM)

Henry Ford Allegiance AIM program provides palliative care to relieve pain, treatment side effects, anxiety and other symptoms of long-term illness. It also offers emotional and spiritual support and comfort.

Care plans are designed to maximize quality of life in later stages of conditions, such as the following:

  • Heart failure
  • COPD, emphysema and other lung diseases
  • Kidney disease
  • Diabetes
  • Neuromuscular disease
  • Cancer
  • Dementia

Hospice care

Henry Ford Allegiance Hospice focuses on maximizing quality of life when a cure is not possible. Our caring staff are specially trained to relieve pain, manage the symptoms of advanced illness, and provide emotional and spiritual support to patients and their families.

Henry Ford Allegiance At Home is the only local provider certified by both Medicare and the Joint Commission. We participate in Medicare, Medicaid, most private insurances, worker’s compensation and private pay.

  • If you have experienced two or more of the following symptoms in the last 3 days:
    • fever
    • chills
    • drenching sweats
    • new cough
    • shortness of breath
    • body aches
    • headache
    • sore throat
    • runny nose or nasal congestion
    • nausea/vomiting/diarrhea
