Al Glick Scholarship Application

To apply, complete the application form below.

As part of your application, you must email the required documents as attachments to hfjhglickscholars@hfhs.org. Please include your full name in the subject line.

Required Documents

  • Transcripts
    • Transcripts do not need to be official.
    • If you are currently attending high school or college, transcripts must be from the current semester (PowerSchool not accepted) and must include the name of the school or university.
  • Letter of Recommendation
    • Include one letter of recommendation (cannot be from a family member or relative).
  • Essay
    • Submit a one-page essay (minimum 650 words) describing what inspires you to pursue your healthcare goals and how you plan to make your community a better place through your education and career.
    • Include your reason for choosing one of the following fields: Registered Nurse, Medical Assistant, Surgical Technologist, Social Worker, Respiratory Therapist, or Athletic Trainer.
  • FAFSA Student Aid Index (SAI)
    • Provide proof of your Student Aid Index (SAI) for the current year.
    • Email your FAFSA confirmation page or FAFSA Submission Summary (FSS) that includes your SAI (do not send the full FAFSA).
    • If you need help completing FAFSA, contact the College and Career Access Center at (517) 990-0671.

Your application will not be considered complete until all required documents have been submitted.

Incomplete applications submitted after February 13, 2026 will not be reviewed or receive follow-up.

Are you currently enrolled in the Jackson County Early College (JCEC) program?
Are you currently enrolled at Jackson Preparatory and Early College (JPEC)?
Are you Dual Enrolled?
Do you currently possess a certificate or college degree?
Are you currently enrolled in a college, university, or vocational institution?
If your program is a second admit program such as Surgical Tech, Respiratory Therapy or Nursing have you applied to your program?
If your program is a second admit program, have you been officially accepted into program?
6. Are you receiving any other financial support in the form of scholarships or other financial aid (Grants) to pay for your degree?

Extracurricular activities

We recognize not every community and school district are provided opportunities for extracurricular activities, and some individuals may experience barriers to participating. Please list 1-4 school, community and/or service activities in which you have participated. These may include family commitments such as caring for a younger sibling or ill relative. Other examples may include clubs, fine arts, volunteering, mission trips, youth programs or sports activities. Please include the approximate number of participation years and/or weekly or monthly hours spent on the activity. Also include any leadership roles held as part of the activity, if applicable.

Work Experience

Please list all paid work experience, including self-employment, that you have held. Please start with your most recent work experience, and list the name of the employer, nature of your work (job title), dates of employment and average hours worked per week.
