The Mosaic Clinically Integrated Network is both a partnership and a promise. To us, it brings together physicians, hospitals, and care teams to work in a more coordinated, patient-centered way. To our community, it is a commitment to deliver against the objectives of the Quintuple Aim. Together, with over 4,500 providers, we design care pathways, close gaps, and align around measures that truly matter to our more than 550,000 attributed patients. We share responsibility for better outcomes and smarter use of resources.

Mosaic moves us beyond the traditional, fragmented care model that rewards volume over value. We recognize value-based care as a more sustainable future for everyone. Our value-based contracts support the effort to reduce unnecessary variation and ensure every patient receives the right care, at the right time, in the right setting.

We uniquely understand the impact of social and behavioral drivers of health, and built a clinical integration model that directs resources against the largest determinants of health outcomes. Our foundation also respects the independence and autonomy of each provider while strengthening our collective impact. That’s the heart of clinical integration – and it’s what positions us to thrive in a rapidly evolving healthcare landscape.
