Mosaic Clinically Integrated Network
Welcome to Mosaic Clinically Integrated Network (CIN). At Mosaic CIN, we are dedicated to improving health outcomes, enhancing care quality and reducing costs through a collaborative, patient-centered approach. Serving diverse communities across southeast and south central Michigan with providers in 13 counties, our network brings together healthcare professionals, hospital systems, physicians and community leaders to create seamless and effective care experiences.
We are one of the Midwest’s leading clinically integrated networks, leveraging data-driven insights and innovative clinical tools to drive performance. As we expand, our commitment to value-based care and meaningful collaboration ensures that our providers are recognized for their impact on the lives they serve.
Mosaic CIN is shaping the future of healthcare — one community, one patient, one partnership at a time.
