MI Mind Application for Provider Organizations

Administrative leaders from provider organizations interested in participating in the MI Mind Collaborative Quality Initiative (CQI) are invited to complete our Common Application for population health CQIs. After submission, applicants will receive an email to complete the MI Mind Full Application. The Applications will be processed annually in March and April but can be completed by administrative leaders at any time.

Eligible practices meet the following criteria:

• Serve an adult (18 years and older) population
• Support integrated models of behavioral health and primary care
• Retain Physician Group Incentive Program (PGIP) Affiliated Providers: Primary care physicians with Patient-Centered Medical Home (PCMH) designation, psychologists, psychiatrists

Applicants not selected for the upcoming cohort will be kept on file. The MI Mind team will contact those applicants during the first quarter of the following year to assess interest in applying for next cohort. The MI Mind team thanks provider organization leaders for their interest in joining the CQI.
