Please complete this form to help us determine if you are a candidate for HIFU

To be considered for HIFU, you must have a diagnosis of essential tremor (ET) or tremor-dominant Parkinson's disease (PD) from your primary care doctor or neurologist. You must also be able to answer "YES" to the following questions:

1.  Are you willing to have your head shaved?
2. Are you able to tolerate laying in an MRI machine for 2 hours?
3. Have you tried at least two medications for your condition?
