For Beneficiaries/Patients

The HFACO is comprised of more than 1,800 physicians both within and outside of the Henry Ford Health System to ensure our beneficiaries (patients) have choices regarding who they seek for care within the HFACO list of providers.

You are still free to receive care at any hospital or by any provider accepting Medicare. However, by staying within the HFACO list of participating and preferred providers, you can help lower your health care costs, while also enjoying additional benefits such as telemedicine appointments with your physician, home visits and more.

Staying Within Henry Ford ACO is Good for Your Health

Did you know that when you keep all—or most—of your care within the Henry Ford Accountable Care Organization (HFACO) roster of providers you help your own health? This is because the overwhelming majority of our HFACO providers are connected by a common information platform. This means your HFACO provider can access your health information immediately. Your provider can also view past test results such as your EKGs, bloodwork, mammograms, colonoscopies, biopsies, etc.

Additionally, your HFACO team of providers can provide you with referrals to nearly any specialty. This is because the HFACO has the resources and deep pool of providers from Henry Ford Health System and hundreds of independent providers available to you. Look at this infographic that illustrates the advantages of staying within the HFACO for your care. As always, if you have any questions, please call us at (313) 874-1466 or email hfaco@hfhs.org.
