Find a Doctor Change Requests
The Find a Doctor change process notifies source systems across Henry Ford Health to update a provider’s record. Once the source systems are updated the new information is sent to our Find a Doctor tool twice weekly on Monday and Thursday evenings. To help you with your change requests we have developed a profile optimization tip sheet.
The change request form is used to: add a provider, delete a provider or to request changes to a provider's profile.
Please select from the following options to facilitate your change request:
- If you have a Henry Ford CorpID, please use the Provider Data Update Request in ServiceNow. This is the quickest way to make a change.
- If you do not have a Henry Ford CorpID, use the Find a Doctor Change form.
If your request involves changes in EPIC you will also need to either contact the Help Desk at (248) 853-4900 or via email at helpdesk@hfhs.org and request a ticket be opened for Epic.
The Find a Doctor change request does not replace HR functions related to hiring and terminating and does not update PeopleSoft or Kronos.
