Who do you trust to make care decisions on your behalf? Who will ensure your medical wishes are carried out as you have specified?
Advance Care Planning is an opportunity for you to communicate the type of treatment and medical care that you would or would not want to receive if you are unable to express this yourself.
Sharing your decisions about end-of-life care with your medical team and loved ones can relieve them from the burden of making tough decisions without your guidance in the future.
This class will teach you the facts about advance care planning, advance directives and living wills and will help you start the conversation about these important topics with your family.
