Quality Improvement
The Community Connect Quality Team is available to assist offices with the following:
- Quality program questions
- Reporting guidance and understanding (i.e. MIPS)
- Performance reports
- Specific measure calculations & workflows to meet measures
- Provider education
Our team partners with Mosaic CIN, Henry Ford Physician Network, and Mosaic ACO to support Community Connect customers and our patients with value-based care across our community.
