The CHTN Quality Team is available to assist offices with the following:

  • Quality program questions
  • Reporting guidance and understanding (i.e. MIPS and Medicaid Meaningful Use)
  • Performance reports
  • Specific measure calculations & workflows to meet measures
  • Provider education

Our team partners with Jackson Health Network, Henry Ford Physician Network, and Mosaic ACO to support CHTN customers and our patients with value-based care across our community.
