Segmented, fragmented and gap-riddled care has (grudgingly) been an accepted shortcoming in healthcare for decades. The HFPN continues to take aggressive approaches to alter the vertical care model by adding programs, services and resources that transform the patient encounter into a horizontal care experience.

Examples of these change include:

  • The use of Epic and Epic Community Connect (at a discounted price) that allows for real-time electronic communication
  • The use of a concierge service for patients with select value-based contracts
  • Options such as Comprehensive Care Centers for chronic/acute patients where they receive an enhanced primary care experience
  • Population Health Management programs to target ways to improve care for specific patient populations
  • Integrated case management that enhances continuity of care
  • Physician relationship management resources
  • Use of the Referring Physician Office
