Biometric Health Screen

You have two options:

Attend an onsite screening at MACI between November 15 and December 18, 2021. Fasting for 12 hours prior is required. The link to schedule an appointment can be found on the MACI homepage: as well as in an enrollment email from It’s Your Life (IYL) if you participated in the program in 2021. Masks are required for onsite screens and will be provided for you.


Have your screening done at your physician's office and return the Physician's Results Form to IYL by fax or place it in the drop box at MACI Corporate Services. We will accept blood work, blood pressure, weight and height values from January 1, 2020 through December 18, 2021. The deadline to return this form is December 18, 2021. A Physician’s Results Form can be picked up at MACI Corporate Services and is attached in the enrollment email from IYL if you participated in the program in 2021.

Health Coaching Sessions

Health coaching sessions will be held in February, May and August 2022. Instructions to sign up for coaching sessions will be sent via email. Participants must complete all three sessions.

All MACI associates and spouses enrolled in Care Management (CM) will be contacted by the Care Manager via phone.

Preventive Health Services

Complete age and gender appropriate preventive health services by September 30, 2022.
