Complete Your Health Screening

Please submit physical and health screening results using the Physician Results Form. Forms available from It’s Your Life staff or R.W. Mercer HR staff. Health screening information from April 1, 2022 or later will count for the program.

Deadline for completing both the Health Assessment and the Health Screening is March 31, 2023.
