How to access your medical records through MyChart
For questions about accessing MyChart call (313) 876-7951.
For medical records from former Ascension locations:
- Starting November 8, 2025, only medical records from that date forward will be available in MyChart for former Ascension clinic offices.
- For records dated before November 8, please submit an Online Records Request or complete a paper Medical Records Release Form.
How to request copies of your records
Medical records can be released to you or someone you authorize. Processing can take up to 30 business days, and fees may apply.
Online medical records requests
To request records electronically online, you’ll need:
- A valid email address
- A cell phone
- A driver’s license or government-issued photo ID

Print a paper Medical Records Release Form
If you prefer not to use the online form you can print out a paper medical records release form:

Required Information:
- Patient’s full name, date of birth, address, phone (include previous names)
- Recipient name and address
- Specific records requested (location, provider name, type of report, dates of service)
- Signature of patient or legal representative. Authorizations signed by a representative must contain a copy of the guardianship papers or power of attorney.
- Date of signature
Requesting deceased patient medical records
To request a deceased patient’s medical records, complete both a medical records release form and the deceased patient medical records form. Both forms are available at the link below:

How to get birth and death certificates
To obtain a copy of a birth or death certificate contact the county clerk’s office in the county the birth or death occurred or from the Michigan Department of Health and Human Services.
