How To Submit Medical Records

In order to provide the best possible care, we need to learn about your medical history. We require all international patients to send us copies of medical records and test results relating to the medical condition we would be treating. All records must be in English.

Submitting records

Once you begin the process of becoming a Henry Ford patient, we will send an international registration form to the email address you provide to us. We’ll need information to complete the registration, including address, date of birth, telephone number, etc.

Our concierge professionals will assign you a unique medical record number. Once you have a medical record number, we will send a secure link for submitting electronic copies of medical records. International patients are responsible for a $1,000 fee for reviewing medical records. This fee is nonrefundable. However, if you decide to travel to Henry Ford for care, we’ll subtract this fee from the total cost of care.
