Video Upload Request Form

Use this form to request video uploads for:

• HenryFord.com
• Blog
• Social media channels
• YouTube
• Internal distribution

Public-facing videos will be uploaded to the Henry Ford Health YouTube channel, Freshpaint or both, depending on how the video will be shared:

• Videos that will be shared on HenryFord.com will be uploaded to Freshpaint. A Freshpaint ID will be provided for use in a separate web request for inclusion on HenryFord.com. These videos will also be uploaded to YouTube to support public viewing and sharing.
• Videos not shared on HenryFord.com will be uploaded to YouTube. A YouTube URL will be provided. This includes videos shared through social media, external communications, and other public-facing channels.

Internal videos will be uploaded to the SharePoint Online (SPO) Video Center and should not contain content intended for public audiences.

Important Notes

• Please allow a minimum of two weeks for your request to be completed.
• Videos intended for HenryFord.com require a separate Web Request submission after the video link has been provided.
• Multiple videos require separate submissions.
