Michigan has seen a historic almost 50% drop in overdose deaths in recent years. Nationally, we have seen impressive declines in cardiovascular and cancer deaths. US life expectancy has hit a new high. In internal medicine residency, I have learned about and seen groundbreaking treatments for diabetes, cancer, hepatitis C, and HIV become widespread. These are major if partial wins. The health of our society and the success of our health care system require us to do the hard yet achievable work of extending these gains to people on society's margins. This means the hundreds of our neighbors who sleep outside in Detroit alone. Exposure to the elements leaves people acutely vulnerable to frostbite, painful wounds, and addiction. Millions of our fellow Michiganders who struggle to afford health care and housing may just be one stroke of bad luck away from the same fate. In the clinic and on the wards, we see the full spectrum of people struggling to maintain their health and housing. With our limited arsenal, we link arms with our patients in their fight against precarity. Street Medicine is a movement that takes this work one step further. I am so honored to be the inaugural fellow in Street Medicine at Henry Ford and CHASS to learn how to organize and deliver excellent care to rough sleepers and honor their dignity by meeting them where they are and on their own terms.
