2022 Program Requirements

Complete the Enrollment Waiver and WebMD Online Personal Health Assessment from March 1st – March 18th.

Biometric Health Screening

Your enrollment email from It’s Your Life will indicate if you are required to have a health screen this year.

Option 1: Onsite health screen at Orbitform in early April. Human Resources will schedule you and send you an invite. You are required to fast for at least 12 hours before (water and medications are allowed). A mask is required and will be provided for you. You will be excused from blood work if pregnant or nursing.

Option 2: Have your screening done at your physician's office. A Physician’s Results Form will be attached in your enrollment email from It’s Your Life. We will accept blood work, blood pressure, weight, and height values done since April 15th, 2021. The deadline to return this form is April 15, 2022.

Participate in Health Coaching Sessions

Health coaching sessions will be held in late April and October and you will be alerted via email.
