Online Surveys

A research program works by gathering lots of information over time. To be successful, we need you to take a little time to answer future surveys. All of the surveys are available in both English and Spanish. Right now, there are 3 additional surveys that will become available in your participant portal 90 days after you consented to join the program.

These surveys include:

  • Health care access and utilization
  • Family history
  • Personal medical history

Your survey answers are important for two big reasons: They tell us about your health experiences, and they can help researchers discover new ways to treat people or keep them healthy. As always, your responses are confidential.

Over time, more surveys will be added for you to complete.  When this happens, you will receive email notifications from the portal and possibly emails and phone calls from Henry Ford All of Us staff. 

Login into your account for new surveys

To complete surveys through the All of Us app on your mobile phone:

  1. Download the All of Us app
  2. Sign in by entering the email address or mobile phone number you used to create your account.
  3. Enter the password you used to create your account.
  4. When new surveys are available, you will see this notification on your app icon:
  5. Open the app.
  6. From the dashboard, click “View All” next to “Complete Your Surveys.”
  7. Follow the instructions to complete new surveys.
  8. When finished with each survey, click “Submit.”

To complete surveys on the web:

  1. Go to JoinAllofUs.org and click “Log In” at the top right side of the page.
  2. Enter the email or mobile phone number you used to create your account.
  3. Enter the password you used to create your account.
  4. Click “Sign In.” Once signed in, you will see the dashboard. Here you will be able to answer surveys.
  5. Start the surveys. They are listed in the first section under “Complete Your Surveys.”
  6. When finished with each survey, click “Submit.” You may also access your own data, sync a fitness device, and change your settings (example: updating your phone number).
