Patient Advisor On-boarding
Thank you for participating in the Patient Advisor Welcome Workshop! We are excited to have you join the Patient Advisor program and are eager to get you involved. Please complete the steps below to finalize your on-boarding into the Patient Advisor program.
Please note: The first step is required to become a Patient Advisor.

Step 1: Submit Forms
Fill out the required Patient Advisor Forms here.
- Photo Release Form
- Gives permission for HFH to take photos for internal use (newsletters, e-blast, etc.)
- Please let PERC know if you do not sign
- Consent Form
- This consent form is an official document that outlines information on a study and/or program and is intended for potential participants (such as you!) to decide whether to move forward and participate or not based on an explanation of the study/program. While the Patient Advisor Program is not a “research study or project,” the program does provide opportunities for Advisors to participate in research studies such as surveys, focus groups, and interviews. When we refer to “research study” and “research project” in the consent form, we are referring to the Patient Advisor Program. Your signature on this form is required for participation in the Patient Advisor Program.

Step 3: PlacementCarefully review the descriptions of active placements in the Patient Advisor Program in the form below. If you are interested in an available placement, select the placement and submit the form. If you don’t see a placement that interests you, please fill out the last box of the form to share an area of interest. Additionally, the PERC team will be communicating new placements and opportunities often via email and the monthly E-Blast.

- Engagement Events
- Throughout the year, the PERC team hosts engagement events for Patient Advisors. Engagement events will be communicated via the monthly electronic newsletter. You are automatically subscribed to our communications list upon joining the program, so all you need to do is check your email each month to stay in the know about events within the program!
