Worksite Health and Well-Being: Program Request Form

Thank you for your interest in our health and well-being programs. If you have questions about scheduling a program, please contact the Center for Health Promotion and Disease Prevention at (313) 874-3095.

Program descriptions

  • Freedom From Smoking® (20 participant maximum): a virtual, 8-session group program led by a Certified Tobacco Treatment Specialist, who provides guidance and support to participants who are trying to quit.
  • Individual Coaching for Tobacco Dependence Treatment: Certified Tobacco Treatment Specialists provide six months of one-on-one coaching and support to participants to guide them through the process of quitting.
  • Nutrition for Well-Being (20 participant maximum): a virtual, 3-session group program, led by a Certified Health Coach who provides basic nutrition education and support to participants who want to improve their diet.
  • Sleep for Well-Being (20 participant maximum): a virtual, 3-session group program, led by a Certified Health Coach who provides education and support to participants who want to optimize the quality and quantity of their sleep.
  • Exercise for Well-Being (20 participant maximum): a virtual, 3-session group program, led by a Certified Health Coach who provides basic physical activity education and support to participants who want to improve on their physical activity.
