Sports Physicals - Henry Ford Jackson

Henry Ford Sports Medicine - Jackson is excited to conduct sports physicals for students planning to participate in athletics during the 2026–2027 school year. All locations are open to students from any school district needing a sports physical. No appointment is necessary.

Students must bring:

• The sports physical and waiver forms downloaded from our site, printed, completed and signed by a parent.
• Cash or check payment of $20. Payment is accepted at the time of the sports physical.
• Please note, athletes who are not accompanied by parents can only be seen by the physician if all forms are completely signed.
• The schedule of available locations and times for sports physicals is below. Choose the location that best suits you.
