Remote Patient Monitoring
Henry Ford e-Home Care’s Remote Patient Monitoring is a personal, in-home monitoring program that combines in-home technology with home health services to help patients better manage their chronic conditions. Participation in our remote patient monitoring program may help prevent avoidable visits to the emergency department and hospitalizations while improving your quality of life.

Who may benefit from Remote Patient Monitoring?
The RPM program is not limited or exclusive for certain people, but the following are some examples of chronic conditions that we routinely monitor remotely:
- Congestive heart Failure (CHF)
- Chronic Obstructive Pulmonary disease (COPD)
- Pulmonary Hypertension
- Severe Hypertension w/ Hospitalization
- Cardiovascular Surgery (CABG, Valve repair/replacement)
- Heart/Lung/Kidney transplant
- Oncology actively treated with chemotherapy
- Atrial Fibrillation
- COVID
- Diabetes
Patients with chronic illnesses such as heart failure, chronic obstructive pulmonary disease (COPD), diabetes or high blood pressure track their vital signs and conditions daily on easy-to-use wireless devices. Patients are also prompted to answer questions about their health and how they are feeling. The information is then transmitted electronically to the RPM team who looks for changes in the patient’s status or results that fall outside normal limits. The RPM team will monitor results and follow up with a phone call and will contact the patient’s home health nurse or physician if needed.

How does Remote Patient Monitoring work?
Cellular devices are delivered to the patient’s home and interface with the Electronic Medical record EPIC through MyChart. Patient answers disease specific survey questions that assess how they are feeling and if they are experiencing any symptoms. Henry Ford e-Home Care’s Remote Patient Monitoring team assess any abnormal values, reviews trends, and reaches out to patient via telephone and/or video visits when necessary.
