Walk-in Sports Physicals

Presented by Henry Ford Pediatrics

Henry Ford Pediatrics is excited to conduct sports physicals for students planning to participate in athletics during the 2025–2026 school year. All walk-in locations are open to students of all ages from any school district needing a sports physical. No appointment is necessary.

Students must bring:

• Sports Physical Form
• Cost is $30.00
• Payment is accepted at the time of the sports physical. Please note, athletes who are not accompanied by parents can only be seen by the physician if all forms are completely signed.

Forms required for visit

Please print this form and have it completed before attending the sports physical. The physicals cannot be conducted without it.
