At Henry Ford, we understand how difficult it can be to manage the tasks of admitting a loved one into hospice care during such an emotional time. We therefore strive to make the process as simple and easy as possible for you and your family.

Admission to hospice care

In most cases, a patient’s doctor makes the request or provides a formal referral for the patient to be admitted into hospice care. Within 24 to 48 hours of the referral, a Henry Ford hospice coordinator will contact the patient or the patient’s family to meet and to complete the necessary consent and insurance forms.

The patient will then start to receive hospice services within a day or two of the referral or in urgent cases, hospice services will begin immediately.

Receiving hospice care

The first visit by the hospice nurse case manager will assess the patient's needs and assist with obtaining any medical equipment that may be necessary for safe home care. At this time, the hospice staff will also develop an individual care plan and set up a schedule for home visits by team members.

Throughout hospice services, the Henry Ford hospice team works closely with the patient’s doctor to ensure that the medications and therapies included in the patient’s individual care plan are keeping the patient comfortable as possible. The patient’s plan is frequently reviewed so that if changes need to occur, they are done so promptly.
