Before going home, our Henry Ford at Home staff will provide one-on-one private consultations to patients and their families to answer questions and ease concerns about at-home care. If products are needed for the continuation of care, each Henry Ford at Home site has a wide selection of home medical products and self-care items that patients and their families may purchase or have delivered to their home before leaving the hospital.

Henry Ford at Home’s seamless integration of services begins even before you leave the hospital. Arrangements for any of Henry Ford at Home’s services may be coordinated on site prior to discharge at any Henry Ford at Home location inside Henry Ford Macomb, West Bloomfield or Wyandotte hospitals.
