Community Health Needs Assessment
As a nonprofit Healthcare system, Henry Ford conducts a Community Health Needs Assessments (CHNA) every three years to identify the most significant health needs of the communities we serve. These needs are identified using information from many sources, including local and state data and input from many community partners and stakeholders. After identifying and prioritizing the health needs in the communities we serve, strategies are created to address these health needs in Implementation Plans.
