Community Health Needs Assessment

As a nonprofit Healthcare system, Henry Ford conducts a Community Health Needs Assessments (CHNA) every three years to identify the most significant health needs of the communities we serve. These needs are identified using information from many sources, including local and state data and input from many community partners and stakeholders. After identifying and prioritizing the health needs in the communities we serve, strategies are created to address these health needs in Implementation Plans.

Our 2022 Community Health Needs Assessment (CHNA) can be accessed here. Implementation Plans will be developed to address the priorities outlined in this needs assessment, and will be posted in May 2023.
