Community Health Needs Assessment

As a nonprofit Healthcare system, Henry Ford conducts a Community Health Needs Assessments (CHNA) every three years to identify the most significant health needs of the communities we serve. These needs are identified using information from many sources, including local and state data and input from many community partners and stakeholders. After identifying and prioritizing the health needs in the communities we serve, strategies are created to address these health needs in Implementation Plans.

Our 2019 Community Health Needs Assessment (CHNA) can be accessed here. The CHNA and Implementation Plans are updated every three years. Updated Implementation Plans addressing the needs identified in the 2019 CHNA will be published in May 2020.
